AS = Academic Standing MG = Midterm Grade
BS = Behavioral Standing TFG = Tentative Final Grade
1PG = First Prelim Grade FFG = Final Final Grade

Phase II

The performance of the trainee in the SFP shall be rated following the prescribed Assessment Form (See Annex C).

Phase III

The Revalida shall be rated based on the Oral Presentation (50%) and the Quality of Output (50%).

Overall Rating

Each Phase will be given the following weights:
Phase I : 45%
Phase II : 45%
Phase III : 10%

Formula : P1 (.45) + P2 (.45) + P3 (.10) = AOR

Legend : P1 = Phase I P2 = Phase II P3 = Phase III
AOR = Accumulated Overall Rating

Course Evaluation

Pre- and Post-Evaluation shall be administered before and after each Module and the Training as a whole. Appropriate guidelines and rating forms shall be developed to evaluate the performance of the trainees during the field immersion, as well as the quality of outputs they will prepare.

VIII. INSTITUTIONAL ARRANGEMENT

CBCOC-BMT

1. Serve as the fostering institutions for the MTP. In such capacity, they shall primarily provide the material, moral and spiritual needs of the trainees.
2. Formulate and implement the MTP policies and guidelines.
3. Coordinate with other concerned institutions relative to the Program.

PBC

1. Initially provide the facilities needed for the conduct of the MTP.
